Responding to the needs of West Virginians has always been my top priority.  My staff is available to help West Virginians who may have problems with federal government agencies, including the Social Security Administration, Department of Veterans Affairs and the Internal Revenue Service.

The Privacy Act of 1974 (Public Law 93-579) requires federal agencies to obtain written permission from you before releasing any information about you to anyone.  Therefore, I will need your written signature on this PRIVACY ACT RELEASE FORM before I can intervene with a federal agency on your behalf.  If you have a printer, print this form out, complete it, sign it, and mail or fax it to one of my offices listed below. If you do not have a printer, you may call to request a Privacy Act Release Form to be mailed to you.
